Instructions: Please submit this form to our booking department at the contact listed above. Once the above information is received, you will be forwarded an honorarium request. Once a budget (including honorarium and traveling expenses) is agreed upon for this booking request, it will be reviewed by management for approval. After which, a contract will be forwarded to you outlining all details. Please note we require a 50% deposit of the contracted amount to solidify the request. Travel includes flights, hotel and local ground transportation for requests outside of the Virginia area. All travel accommodations must be booked at least 30 days prior to the booking request date.
